Scholarship Waiver

INSPIRE WOMEN’S RELEASE OF INFORMATION WAIVER In applying for a scholarship from the Inspire Women Organization, I give permission to use any of my information to raise funds for Inspire Women, even if I am not selected as one of the scholarship recipients. I also understand that if I accept an invitation from Inspire Women to share my personal testimony, this does not ensure me of any scholarship funding. I understand that my application will be reviewed by an independent panel that is put together specifically to select scholarship recipients. Furthermore, if I am awarded a scholarship, I give permission to Inspire Women to obtain my academic records from any institution I have attended or will be attending. I understand these records will be used by Inspire Women for the purpose of validating my G.P.A. which must be 2.5 or above, in order for me to receive funding from Inspire Women. I hereby give Inspire Women permission to capture my name, likeness, image and/or voice in photographic (including but not restricted to individual pictures, small groups pictures, team group pictures), audio recordings and/or quotations(including written testimonies), audiovisual, digital or any other form of medium (the “Media Materials”); and to use the Media Materials for any purpose (including any and all business, educational, professional, or fund raising activities), in any manner, in whole or in part, and in its sole discretion, including but not limited to the Media Materials being used by an individual or entity other than Inspire Women. I understand and agree that the Media Materials are the property of Inspire Women; the Media Materials may be released to the public by Inspire Women and, when previously authorized by Inspire Women, Shelton Interactive. Such public release shall be as and when authorized by Inspire Women and may be provided to and distributed through the news media, including the broadcast, print and/or digital/social media. I hereby waive any right to inspect or approve the Media Materials; any printed or digital matter that may be used in conjunction with the Media Materials; and the eventual use of the Media Materials in digital, social media or print use. In addition, I hereby waive all rights, interest, or claims for payment or other compensation in connection with the Media Materials, including any use, copying, distribution, publishing, display, exhibition or release of the Media Materials by Inspire Women; and release Inspire Women from any and all liability in connection with the Media Materials. With my signature below, I hereby release and waive any claim that I may have, now or in the future, against Inspire Women, all event sponsors and volunteers, and any individuals having any connection to Inspire Women events, for injuries, illnesses, accidents, property losses or hazardous health conditions I might suffer as a result of my participation in such events. I also agree that my telephone number and email addresses may be given to Inspire Women’s staff and volunteers. This is a perpetual and worldwide waiver and grant of rights.
